MY APPROACH

During your treatment, you will have the full attention of your physical therapist for a full 60 minute session. No assistants, no unsupervised time doing exercises, just 60 minutes of dedicated physical therapy with an expert.

These in-depth sessions are designed to ensure you are treated in the minimum number of sessions, understand the condition you are suffering from, know how to do the exercises on your own, and can get back to doing what you love as soon as possible.

I provide physical therapy services in your home or at one of several locations throughout NYC.  For in-home treatment, I will bring all of the necessary equipment, including a treatment table, right to your door.

Most NYC physical therapy clinics double-book, hand you off to an aide, and bill your insurance for a half-finished session. On Point Physical Therapy does the opposite. Every minute is with Ariel Osharenko, PT — the same physical therapist, every single visit. Every plan is built around your specific injury, your sport or activity, and the life you actually live.

I do not accept insurance directly. Most patients get reimbursed 50-80% of their session cost by their PPO plan — I provide a complete superbill (an itemized receipt with the correct CPT codes) that you submit to your insurance company. Many patients also use HSA or FSA funds, which are eligible for physical therapy.

What you get in return: undivided 60-minute attention, the same physical therapist every visit, no rushed protocols, no insurance-driven session-length limits, no pre-authorization headaches, no surprise bills, no "your benefits ran out." Just real physical therapy until you're done.

From the day after surgery through return-to-sport testing. The ACL rehab program at On Point follows the latest JOSPT clinical guidelines for ACL reconstruction rehabilitation, integrated with force plate testing, Y-balance assessments, and progressive strength benchmarks. Typical timeline: weekly sessions for the first 12 weeks, tapering to bi-weekly and monthly as you progress. In-Home Physical Therapy Across NYC

This works exceptionally well for post-surgery patients who can't easily travel, busy professionals who lose two hours every time they leave the office, new parents, and patients who genuinely heal faster when treated in their actual environment. Physical therapy doesn't stop at "no pain." It stops when you're outperforming what you could do before the injury — and you have objective measurements to prove it. Force plate jump testing, Y-balance, return-to-sport criteria from the latest peer-reviewed literature. Post-Surgery Recovery

ACL reconstruction, meniscus repair, rotator cuff repair, total knee replacement, total hip replacement, shoulder labrum, hand and wrist surgery. The first 6-12 weeks set the trajectory for everything that comes after — and that's where most patients lose ground because of rushed insurance-driven appointments. One-on-one 60-minute sessions are exactly what early-phase post-op rehab needs.

Runner's Knee, Shin Splints, IT-Band Syndrome

NYC has more runners than anywhere else in the country and a fraction of the physical therapists who actually understand running biomechanics. I run myself — and I treat runners training for the NYC Marathon, the Brooklyn Half, the NYC Half, weekend Central Park runners, and casual joggers. We look at your gait, your cadence, your shoes, your weekly mileage, your strength asymmetries — not just where it hurts.

Back & Neck Pain

Desk-worker cervical pain, herniated lumbar disc, post-surgical spine recovery, sciatica, thoracic outlet, headaches with cervical origin. Manual therapy, targeted strengthening, and ergonomic adjustments that actually work in a small NYC apartment with a real desk setup. Most "I have back pain" doesn't need surgery — but it does need someone who'll spend a full session looking at the actual problem.

Shoulder Rehab

Rotator cuff tears (full and partial), impingement, frozen shoulder, post-surgical labrum repairs, AC joint, throwing athletes, swimmers, climbers. Shoulders are commonly misdiagnosed and over-treated — I'd rather spend a full session getting the diagnosis right than running the generic protocol everyone gets.

Pelvic Floor & Post-Partum PT

For new moms across LIC, Astoria, and the rest of NYC — diastasis recti, pelvic organ prolapse, return-to-running after pregnancy, c-section recovery, postpartum core rebuilding. I work directly when the issue is musculoskeletal, and I refer to specialized pelvic floor PTs when the issue calls for them.

How does out-of-network physical therapy reimbursement work?

You pay me directly at the time of service. I give you a superbill — an itemized receipt with the correct CPT codes (typically 97110 therapeutic exercise, 97140 manual therapy, 97530 therapeutic activities). What conditions do you specialize in?

ACL rehabilitation and post-surgical knee recovery are the largest specialties, followed by runners' injuries (knee, shin, hip, foot), shoulder rehab, back and neck pain, and post-partum PT. How many sessions will I need?

Depends entirely on the condition. ACL rehab typically runs 6-9 months (1-2x per week tapering down). Runners knee or non-acute back pain might be 4-8 sessions. Post-surgical shoulder is usually 12-16 weeks. We'll set realistic milestones at session one and adjust as you progress.
